Southwest Region Report November, 2020

​I spoke to several LC’s from different areas and many hospitals in the southwest region. Although there were not many new things going on due Corona and our new way of life, they are all seeing inpatients and out patients and have managed to stay protected and well with proper safety measures in place. Some have had an increase in patients. WIC offices are doing a mixture of things. Some are seeing moms in office or via zoom. But some are still not seeing moms in clinic at all. Terri Rutz IBCLC at Children’s mentioned the Southwest breastfeeding coalition meeting that can be joined on zoom next Monday at 6 pm. If anyone is interested, the information can be found below:

From Amber Clary. Mercy Health West Hospital Donor Breastmilk Program is still going. The hospital agreed to pay for the Donor Breastmilk for their babies after the grant money (from OLCA last year) was depleted. 
 
From Laura Atkinson. It continues to be busy here at Wright-Patt Medical Center. My position was immediately converted to essential to support breastfeeding families at the start of the Covid-19 restrictions.  This included all inpatient and outpatient families.  So no change in services and I had an increase.  Screening processes were put in place for everyone’s safety. Parents and their babies continued rooming in together.  The leadership was extremely supportive. During the beginning of the pandemic breastfeeding initiation rates increased to 100% in April and May!  Our 2020 Physical Year just ended and the stats show a 95% BF initiation rate with 82% exclusive (formula per mother’s choice excluded).  The 18% supplemented were medically indicated.  Hypoglycemia being one of the reasons. The Pediatricians have initiated using oral glucose gel if expressed colostrum is not available.  So more to come on that outcome.  Tricare covers banked human milk (HMBANA) for sick infants.  We are exploring having banked milk available, but we are a Level 1 and infants needing a higher level of care transfer to Dayton Childrens’ Hospital where banked human milk is available.  Community BF support went mostly online and continued to be an important resource for our families, so thank you to all.  I’d like to add that if a family can access the base I can be a part of their BF support. 

From Anita Coleman, IBCLC. Dr. Chris Coleman.  Yes we are still working in our office helping parents and their babies, as well as dental patients. 
Being a dental office we are used to thoroughly cleaning everything in between patients and using proper PPE protocol for safety. 
I contact parents before their appointment sharing a video that I made to describe they are experience in our office and the procedure. 
Parents are asked to stay in their car until it’s their turn to come in. They must wear their mask coming in and leaving as well as in our office -until the mouth is needed! Temperatures are taken and a questionnaire is filled out as to possible contact with an infectious person. 
I work with a parent before the procedure, and Dr. Coleman discusses it with them and answers all their questions. We invite the parents to stay in with the baby during the procedure and then I helped them nurse again afterwards in our lactation room.
I send a video with the post-Frenotomy stretching exercises, and reminder information and I offer my phone number if they have any questions. 
Then at about a month out I follow up with a text to check to see how things are going.

From Linda Barrett, IBCLC, at WIC and LLL Leche leader of Centerville. Not a whole lot happening on the La Leche League front. Groups are having either Facebook Live or Zoom meetings. Info can be viewed on the individual Group Facebook pages. Attendance has been light. Those interested can go to the La Leche League of Ohio website to view the meeting information for the Ohio Groups. 
 
Wright State University's Boonshoft School of Medicine has been requiring an interpersonal experience for their students. Students have gone to La Leche League of Centerville and Breastfeeding USA meetings. Some have gone to Preble Co WIC with Marilyn Wilson to observe. 
 
Public Health Dayton Montgomery County WIC Program is in the interview process for Peer Helpers.

Currently, moms and babies are being certified by phone and come in only to have their WIC Nutrition Card loaded.  Breastfeeding services are currently by phone or text only. Pump loans and issuance are still being conducted following social distancing requirements. 
 
Premier Health Miami Valley Hospital's Lactation Clinic and Kettering Medical Center's First Steps at Southview Hospital continue to support breastfeeding moms and are accepting appointments for one-on-one consultation as well as telephone help.
 
Greater Miami Valley Breastfeeding Coalition (GMVBFC) is currently meeting via Zoom once per month. If anyone is interested in being on the Yahoo notification list is welcome. If Yahoo is not an option, they can email Linda Barrett at lbarrett57@tgmail.com to be included on our email list.

Southwest Region Report September 2020
 
Melissa’s daughter Celina, had baby Josiah. He experienced slight issues while going through the birth canal. His C1 vertebrae was rotated which caused major breastfeeding issues. Pain, creased nipples, and discomfort for the baby. Celina had to exclusively pump. After continued visits to the chiropractor, they have successfully resumed painless comfortable breastfeeding. 
 
Montgomery county WIC offices are still not allowing Breastfeeding staff to see moms for consults or breastfeeding assistance. Only for issuing pumps which is a quick issuance and moms are directed to contact the pump company or refer to you tube videos for assistance. 
 
At Southview Hospital in Centerville, Ohio, Covid moms and babies are kept together. There is more interest in breastfeeding.
 
At Miami Valley Hospital in Dayton, Ohio, moms can refuse test and are not assumed positive and can still stay with their babies.
 
At Wright Patt Medical Center in Dayton, Ohio as of July, Bf rates were 100 percent. Moms continue to breastfeed or pump for past a year.
 
The Good Neighbor House in Dayton, where Dr. Notestine does frenotomies for the underserved is now accepting Medicaid and Caresource patients.
 
At Children’s NICU in Dayton, Dr. Velasco has a system now where it can be inputed how many times a baby breastfeeds during the day and in addition IV fluids any intake by bottle. Before and  after feeds are also tracked.
 
Public Health Dayton and Montgomery County posted videos on the facebook page discussing what  the BAM theme meant to them. Three African American breastfeeding staff members reflected on breastfeeding and black breastfeeding week.
 
Darke county la leche league hosted a virtual live love latch event. Digital goodie bags, door prizes, expert talks, and children story time videos were provided.
